Social Services

The Social Work Department provides support and information to patients and their family members in the following areas:

  • Discharge Planning Assistance is provided for any services or equipment patients may need when they are ready for discharge from the hospital. Those services may include but are not limited to: home health care, skilled nursing facility care, medical equipment needs, community resource planning, and help in the decision making process. A Discharge Planning Guide provides a list of some of the most frequently used agencies and programs.

  • Mental and Emotional Health Resources Counseling services include: crisis intervention; individual counseling for needs related to health problem such as anxiety, depression or adjustment problems; referrals to appropriate mental health counseling services; and,

  • Information & referral for support groups located in the community. Information & Referral Services Social workers can help provide information about local services and help in locating similar information and referral services in other communities.
