About Care Management
Pullman Regional's Care Management team is focused on helping patients stay connected to their care between clinic visits. This service provides personalized, wrap-around support to patients who may have complex medical conditions, barriers to accessing care, or need help managing ongoing health needs. Their work ensures patients receive the right care, at the right place, at the right time, while helping ease the load on our providers.
What Care Management Offers:
- Free, patient-centered care coordination
- Help managing chronic conditions (like diabetes or high blood pressure), through phone calls, MyChart messages, or at the patient’s appointment
- Assistance with prescriptions, appointments, and follow-up care
- Collaboration with providers to improve outcomes and confidence in care
MEET OUR TEAM
Shannon Hatley
RN, BSN
Shannon Hatley is a Care Manager with more than 40 years of nursing experience, including 36 years serving patients and families in Whitman County. Her background spans hospital nursing in Colorado and Washington, 26 years in Public Health, and 10 years at Pullman Family Medicine offering care coordination, Annual Wellness Visits, and patient education. Shannon now supports patients through Pullman Regional Hospital’s Care Management Program, focusing on empowering individuals to feel in charge of their health.
Mary Brentlinger
RN, ASN, BS, MS
Mary Brentlinger brings more than 45 years of nursing and healthcare experience to her role as a Care Manager at Pullman Regional. A retired Chief Hospital Corpsman, Mary served 17 years in the U.S. Navy in various roles ranging from pharmacy technician to managing outpatient military clinics. Outside of the military, Mary's experience includes long-term care pharmacy work, dementia-focused skilled nursing, hospice case management, and leadership as a Clinical Manager and Executive Director of Hospice on the Palouse. She spent five years at Palouse Medical providing care management, Annual Wellness Visits, patient education, and clinical support, and she now serves patients across Pullman Regional’s Care Management Program.
Sofia Cervantes
Sofia Cervantes joined Pullman Regional’s Care Management team after starting her career at Palouse Medical/Pullman ReadyCare, where she supported patients with check-in and other registration details. She now assists nurse care managers and palliative care staff while connecting patients with chronic conditions to appropriate resources and primary care providers. A Washington State University graduate in Psychology with deep roots in the Pullman community, Sofia enjoys listening to patients and helping them feel supported in their care.
